Appointments
While we try to keep waiting times to a minimum, emergencies may occasionally disrupt our schedule.  If a significant delay in the doctor’s schedule occurs, his/her assistant will try to notify you so that you will have the option of rescheduling your appointment or, if it is possible, waiting until the doctor is available.  If you are unable to keep a scheduled appointment, please notify us at least 24 hours in advance so that we may accommodate other patients.  If no notification is given by the patient, a fee may be assessed. If you are more than 15 minutes late for a scheduled appointment, it may be necessary to reschedule you, so as not to inconvenience other patients. 

At the time of your first office visit, please bring the following with you:

• Completed registration forms, either mailed to you or available to download
• Health insurance card and photo ID
• If scan (MRI, CT, etc.) was done, please bring report and films to help in diagnosing or treating your problem

Plan to arrive at least 20 minutes prior to you scheduled appointment time in order to complete the necessary forms if you haven’t completed them in advance of your appointment. 

Procedure for Prescription Refills

• Call your pharmacy for any and all refills. Pharmacy will contact our office if necessary.
• Call the offfice 4-7 days prior to finishing out your medications. Some medications must be preauthorized and this takes time.
• Allow 24 to 48 hours for turn around time once a refill has been requested.
• Sample pick up also requires a 24 hour notice for completion.
• Always call ahead to make sure your written prescription or samples are ready before coming into office to pick up.

*Note control #2 drugs can not be called in. They must be picked up or mailed. These medications require a 48 hour notice. Example= Adderoll, etc..
