We at Roanoke Neurological Associates Inc. appreciate the referrals that come to us from physicians and health care providers from around the region. We do require that the referring doctors’ offices call or fax appointment requests for new patients. Therefore, we strive to make the referral process as smooth as possible.

Appointments
When an appointment is needed, please contact our office to schedule an appointment for the patient. If the patient has an insurance that requires a referral please make sure that takes place before the patient is seen in our office.

Worker's Compensation Appointments
If the patient has a Workers Compensation claim, the Referring physician must provide on letterhead the Workers Compensation insurance authorization for the patient to see our physician. The documentation must contain the following:

• Patient’s Name
• Patient’s Date of Birth
• Patient’s Address
• Patient’s Phone Number
• Claim Number
• Date of Injury
• Reason for Visit
• Diagnosis codes and nature of injury
• Billing address for Workers Compensation Insurance
• Claims adjuster name and contact number
